Our ABQ StreetConnect program will be providing prioritized behavioral outreach for more of our vulnerable community members! ABQ StreetConnect is our specialized outreach team whose mission is to identify, engage, stabilize, and house individuals for whom traditional outreach methods have not worked. Our ABQ StreetConnect program, led by outreach director Jodie Jepson, is disrupting a cycle of high system utilization for people experiencing homelessness who have the highest records of utilization of emergency services and care to establish ongoing relationships that result in permanent supportive housing.

The team which was previously made up of four navigators and limited the downtown corridor and UNM Hospital is expanding! In addition to the downtown corridor and UNM Hospital, we will be working with the Albuquerque Police Department to identify and help people experiencing homelessness in the valley and southeast areas of Albuquerque. We will have a total of eight navigators who will be serving 120 people.

Heading Home is proud to offer this type of outreach to more of the Albuquerque community! The type of outreach our team offers is data-driven and we wouldn’t be able to have this level of success without collaboration with the Albuquerque Police Department, City Councilor Isaac Benton, UNM Hospital, City of Albuquerque Family and Community Services, Albuquerque Healthcare for the Homeless, Presbyterian Kaseman Hospital, Blue Cross Blue Shield, and countless others.
