OUTREACH

StreetConnect
Our street outreach team spends every day connecting with people, providing information, guiding them to support services and medical resources for assistance, and assisting them with creating pathways toward housing.

StreetConnect provides housing vouchers and hotel vouchers to clients. Clients are referred by the Albuquerque Police Department (APD), Albuquerque Community Services (ACS), the City of Albuquerque’s Health, Housing and Homelessness department, and partner hospitals.

Dave became a client of StreetConnect six years ago, battling severe behavioral health issues. Through the help of his case manager, Dave overcame his barriers to stability. Dave has been housed for five years, and recently received a housing voucher from the VA.

Shelter & Respite

AOC Men’s Shelter

The Albuquerque Opportunity Center (AOC) is a 56-bed facility for men. The program offers housing case management, job counseling, linkage to resources, and more. Space is on a first come, first served basis.

To reserve a bed, call the Intake Line at 505-344-4340 (1am – 3am & 9am – 3pm). If there is no answer, that means the beds are full for that day. Please try back the following day.

Respite Program at the AOC

The AOC offers a 34-bed respite program for men and women. The program is available only through referral. All guests must be referred through a participating healthcare provider that has an agreement with the AOC. Guests in the respite program are provided three meals a day, case management, and transportation for medical appointments.

Gateway Medical Respite

Gateway Medical Respite is a collaborative pilot program spearheaded by Albuquerque Healthcare for the Homeless and partnered with Heading Home, University of New Mexico Hospital (UNMH), First Nations, and the City of Albuquerque.

The program provides acute and post-acute medical care for individuals who are homeless and too ill to recover on the street, but not sick enough to warrant hospital-level care. Case management is provided to assist clients with the goal of exiting the program to stable housing, decreasing visits the to the ER, and fewer hospital readmissions.

Clients must have a referral from UNMH or Albuquerque Healthcare for the Homeless for admission. For more information, visit the city website or call Healthcare for the Homeless at 505-766-5197.

Staying at AOC provided the stability Sam needed to get a job and maintain his sobriety. With the support and guidance of his case manager, Sam was able to find an affordable apartment. Sam has continued to work, and recently celebrated his two-year anniversary of being sober.

Housing

Housing isn’t just shelter – it’s stability.

Before someone can get a job, focus on school, or improve their health, they need a safe space to sleep. Heading Home oversees subsidized housing programs for individuals and families.
 
Clients in our housing programs receive ongoing case management focused on housing stability, linkages to behavioral health resources and providers, assistance with applications for benefits, and ongoing assistance linked to obtaining affordable, long-term, permanent housing options. Our case managers work with other partners in the community to provide a holistic supportive approach for client stability.
 
Our housing programs require a referral from community partners. Criteria for each of the housing programs varies. If you are seeking housing, please contact your case manager, care coordinator, or other support resource. They will need to submit a referral for you.

Marisella and her two children found themselves homeless after her husband died. They were able to secure housing through one of Heading Home’s programs. The stability allowed her to continue prioritizing the children’s education. Her son with special needs graduated high school in 2025 and received the APS Education Foundation’s Selfless Senior Award for promoting positive change in their lives and the lives of others.

Employment Support

JobConnect

Heading Home’s JobConnect program helps those who are experiencing homelessness get back on their feet and earn an income. Program staff help clients with job search assistance, application guidance, resume building, mock interviews, and securing proper ID and documentation for the hiring process. The program also provides clients funds for training programs, work clothes, shoes, transportation, cell phone data, and more.

Referrals for JobConnect clients come from our shelters and housing programs, and from select Bernalillo County programs. We partner with several organizations, including NM Workforce Connections/Solutions, Salvation Army, Goodwill, and Hiring Hub.

When Carlos arrived at the AOC men’s shelter, he was homeless and unemployed. The JobConnect program helped him refine his job search, identify local opportunities that matched his skills, and prepare him for interviews. His determination never wavered, and within three weeks of joining the program he secured employment. Just five months after entering the shelter, Carlos had saved enough money to move into his own apartment.

SOAR

SSI/SSDI Outreach, Access & Recovery

The circumstances faced by our homeless community members make it difficult to access disability benefits on their own. Barriers include lack of a permanent address, lack of medical records due to inconsistent medical treatment, and symptoms that interfere with cognitive function, making the process even more challenging.

Our SOAR program assists individuals in overcoming these barriers, promoting recovery and wellness through increased accessibility to Social Security disability benefits for individuals who are at risk of or are currently experiencing homelessness, have a serious mental illness, medical impairment, and/or a co-occurring substance abuse disorder.
