A new report that analyzes the recent outcomes of Heading Home’s street outreach program is coming soon and the results are very promising!

ABQ StreetConnect was created in August 2017 “to provide intensive, individualized, housing-focused service navigation to individuals with mental illness experiencing chronic homelessness.” The program has evolved to focus on individuals experiencing homelessness who are the highest utilizers of emergency services, most of whom suffer from severe behavioral health issues.

ABQ StreetConnect provides individualized case management and connects clients with behavioral health services. Staff work to place clients in appropriate housing that may include group homes, assisted living and skilled nursing facilities.
